What to include
Name the company and, where you have one, the product. For a certificate question, include the certificate number from the badge or PDF and say what the live record shows when you look it up. For a procurement pack, say whether you are the buyer or the supplier and which requirement you need mapped. We cannot take payment on this form, and a message does not start an assessment until the assessment page confirms the scope.
Who should I contact about a directory listing?

Use this form for Bronze, Silver or Gold assessment scope, or when a live verification record does not match the certificate number you were given. Look up a certificate on the verify page before writing.
